City of Milwaukee Onsite Nurse Liaison

An onsite nurse liaison through UHC is available to help employees/spouses understand information and follow-up steps from doctor appointments, provide support with chronic medical conditions like diabetes, hypertension and asthma, find the right type of care and address questions or issues regarding care or treatment. This service is FREE for City of Milwaukee employees and spouses.

UHC and OptumRx Monthly Onsite Visits

Representatives from UHC and OptumRx will be at City Hall and DPW Headquarters monthly to meet with employees. Employees can stop by either location at the designated times for a personal meeting to answer questions on claims, prescriptions, issues or general inquiries regarding benefits.

Wellness Your Choice Milwaukee 

Workplace Clinic/FastCare Clinics

Starting July 1, 2018 employees, spouses and dependents (ages 6 and older) with the City’s UHC health insurance will be able to use 5 Froedtert FastCare Clinic locations around the Milwaukee area to receive FREE care. Please see the flyer for more information and FAQs.

Healthy Rewards Program Description

Healthy Rewards is the City’s incentive-based wellness program where participants can earn 3 levels of points to earn a Health Reimbursement Account (HRA) up to $700 if spouse completes the program. Employees and Spouses must complete the Health Appraisal process to be eligible to participate in the Healthy Rewards Program.
