Employees are responsible for keeping their enrollment status, including births and marriages, current through the City's Self Service program at www.milwaukee.gov/selfservice. All employees must have their Employee ID number (6-digits) and a password to access self service. To request or reset a password go to www.milwaukee.gov/rits.
Active City of Milwaukee employees should return their completed form to DER Employee Benefits, City Hall, Room 706 or derbenefits@milwaukee.gov.
If you have questions regarding your benefits, unpaid bills, or problems with service, please call your health, dental or vision plan. DO NOT call Employee Benefits until you have contacted your health, dental or vision plan and are unable to arrive at a resolution. Employee Benefits will attempt to assist you with resolving your problem, but in no case will Employee Benefits attempt to change, question or provide a medical opinion. Document all your conversations with dates, times and names. DER will ask for this information when you call the Employee Benefits Division at 414-286-3184, You can also email your questions to derbenefits@milwaukee.gov
