
Community Health Improvement Plan
Supported by the Mayor, the Board of Health, and the City of Milwaukee Health Department, the MKE Elevate Community Health Improvement Plan (CHIP) responds to priority issues and seeks to create shared goals to address these issues.
Community Health Improvement Plan: The CHA/CHIP Cycle
The MKE Elevate Community Health Improvement Plan, or CHIP, report selects priorities based on data and community input from the Community Health Assessment. Using surveys, photo submissions, and feedback from community meetings, the CHIP organizes the community health assessment data to actionable steps for community improvement. The current CHA/CHIP cycle details are below:

How to Use this Report
Designed to complement and build upon other guiding documents, plans, initiatives, and coalitions already in place, the CHIP is a report that demonstrates how to improve the health of the Milwaukee community. It serves as a vision for the health of the community and a framework for individuals and organizations to leverage resources, build partnerships, and identify their own priorities and strategies.
Community members, students, community-based partners, and healthcare professionals can use this report to:
• Review the priorities and goals
• Reflect on the suggested strategies, and
• Consider how you can participate in this effort
