Community Healthcare Access Program (CHAP)

The City of Milwaukee Health Department’s (MHD) Community Healthcare Access Program (CHAP) is designed to help individuals explore options for affordable health insurance coverage.

The CHAP team is committed to ensuring that everyone in the community has access to the healthcare coverage they need. The program’s Certified Application Counselors, trained under the Affordable Care Act (ACA), guide individuals through enrollment by:

  • Helping explore insurance options
  • Determine eligibility for free or low-cost coverage
  • Find a plan that fits their budget.

    Spanish and English-speaking staff are available on-site. If assistance is needed in a language other than English or Spanish, individuals are encouraged to contact us at 414 286-8620 to arrange translation services.

What programs can the Community Healthcare Access Program assist me in applying for?
 

CHAP employees are Certified Application Counselors under the Affordable Care Act (ACA), trained to assist you with enrollment. While the ACA offers new health coverage options for qualified individuals, CHAP will guide you every step of the way to determine which programs and services you qualify for and what low-cost plans are available.

CHAP can help you determine if you qualify for:

  • Health Insurance Marketplace (private insurance)
  • BadgerCare (Medicaid)
  • Family Planning Waiver
  • Food Share
  • Child Care
  • Express Enrollment for pregnant women and children

CHAP can also provide referrals for enrollment in W-2, WIC, energy assistance, dental services, and free or sliding-fee clinics.

  How do I know if I qualify for free or low-cost health insurance?

 

If you are ready to enroll in a health insurance plan, please have recent financial information available, including tax returns, pay stubs, or other income statements.  Also, be aware of how many dependents you have under the age of 26, as adult children may be eligible for coverage.

Finally, if health insurance is offered as a benefit through your employer, you must document that the cost of that insurance is more than 9.5% of your income. If your work insurance is higher than 9.5% of your income, you may be eligible to purchase insurance through the Health Insurance Marketplace. A Certified Application Counselor can help you enter your information online to see what insurance options are available and check if you qualify for any financial help.

  What is the Affordable Care Act (ACA), and how can it help me?

 

The Affordable Care Act (ACA) provides more options for individuals and families to access affordable health insurance. Key points about the ACA include:

  1. Everyone can get health insurance, even with pre-existing conditions.
  2. During the annual open enrollment period, you can access the Health Insurance Marketplace (www.healthcare.gov) to sign up for affordable, federally-approved insurance plans.
  3. The Marketplace application determines if you qualify for additional programs, such as Medicaid or BadgerCare.
  What if I am already covered by Medicaid/BadgerCare or Medicare?

 

If you are currently covered by Medicaid/BadgerCare, you may receive a letter instructing you to enroll for coverage through the Health Insurance Marketplace. If you receive this letter, it is important to act promptly to avoid losing your coverage.

If you are covered under Medicare (for those disabled or 65 and older), your coverage does not change, and you do not need to use the Marketplace.

  Can I get help paying for my health insurance?

Many people qualify for financial help to lower the cost of health insurance through government assistance. A Certified Application Counselor or enrollment assistant can help you find out if you qualify. For assistance, call 2-1-1 or visit www.healthcare.gov.
