Personal History Questionnaire (PHQ) 

If you have applied recently for the positions of Police Aide, Fire Cadet, or 911 Operator and are still participating in the hiring process you are required to complete a Personal History Questionnaire (PHQ) in order to begin your background investigation.  FPC staff has emailed each candidate a link to a copy of the PHQ but those that cannot locate that link may find the documents here:
