- This form should be completed by all property owners who received an income and expense request letter from our office.
- Submission of form PR-323 is required to preserve your right to object to your assessment under Wis. Stat. § 70.47(7)(af).

When completing forms:
- You may submit multiple forms if you require additional lines.
- Not all the requested information applies to every property.
- Please complete the sections that apply to your property.
- Be sure to include the taxkey and property address on all submitted forms.
Questions and digital form submissions should be directed to your property's appraiser.
