  • This form should be completed by all property owners who received an income and expense request letter from our office.
  • Submission of form PR-323 is required to preserve your right to object to your assessment under Wis. Stat. § 70.47(7)(af).

• When completing forms:
  • You may submit multiple forms if you require additional lines.
  • Not all the requested information applies to every property.
  • Please complete the sections that apply to your property.
  • Be sure to include the taxkey and property address on all submitted forms.
• Questions and digital form submissions should be directed to your property's appraiser.
  • Refer to your I&E request letter for appraiser's email and phone numbers.
