You are required to submit an Agent Authorization form (Wisconsin DOR PA-105) with your assessment objection if:
If the property is owned by an individual but that individual wishes to have someone who is not a listed owner speak on their behalf, they must complete and file the Agent Authorization form authorizing the other person to speak and receive communications on their behalf.
If a property is owned by a business or corporate entity, a valid Agent Authorization form must be on file for us to speak with and send communications to the agent about an assessment objection related to that property. If you are the individual owner of a business, you must still complete and file an Agent Authorization form. If you are the new owner of a business, you must file a new Agent Authorization form with the current owner’s name.
All sections of the Agent Authorization form must be completed for the form to be considered valid. This includes the authorization expiration date in Section 3. If you do not want the form to expire, you should write "Does not expire unless revoked" or similar or similar in the expiration date section.
