Agent Authorization Form
You are required to submit an agent authorization form with your assessment objection if:
- The property is owned by a business, corporate or legal entity (e.g., trust, LLC, Inc.); or
- Someone other than the property owner is to handle property assessment appeal.

If you use the Milwaukee Real Property Appeals web application to submit your appeal, an agent authorization section is provided when necessary, eliminating the need to submit a separate agent authorization form. Note: New appeal submissions using this web application are only available during Open Book.
If an agent authorization form is required, a completed agent authorization form must be attached to the objection form or the objection may not be processed.
The City of Milwaukee asks that a new agent authorization form be submitted for each assessment year. Failure to submit a new agent authorization form will result in a longer processing time for property assessment appeals.
If a property is owned by a business or corporate entity, a valid agent authorization form must be on file for us to speak with and send communications to the agent about an assessment appeal related to that property. If you are the individual owner of a business, you must still complete and file an agent authorization form. If you are the new owner of a business, you must file a new agent authorization form with the current owner’s name.
All sections of the agent authorization form must be completed for the form to be considered valid.
