Wisconsin Death Certificates By Mail

Members of the decedent's immediate family may obtain a certified copies of death certificates. Relationship must be stated (i.e., I wish to obtain a copy of my father's death certificate).   You must also include a copy of your CURRENT ID with the application.   This office has death certificates for ALL Wisconsin deaths occurring 9/2013 to the present and for all City of Milwaukee deaths back to 1869.  Uncertified death certificates usually do not include a cause of death.   If you have a question, please call us at 414-286-3503.

To request a death certificate by mail, write a letter including the following information:

  • Relationship to deceased.

  • Certified death certificates can only be issued to a member of the immediate family, i.e. spouse, child, parent, brother, sister or grandparent; a legal guardian of the person named on the death certificate, a representative who is authorized IN WRITING to act on behalf of the deceased, or to one who can demonstrate that the certificate is needed for the determination or protection of a personal or property right.

  • Full name of person named on the record.

  • Date of death.

  • City and county of death

  • Include a copy of an acceptable form of identification.    These are:

ONE of these:

·         Wisconsin driver’s license

·         Wisconsin photo ID

·         Out-of-state driver’s license or photo ID card

OR

TWO of these:

·         Government issued employee ID card or badge with photo

·         US passport

·         Check or bank book

·         Health insurance card

·         Recent dated, signed lease

·         Recent utility bill or traffic ticket

  • Be sure to sign and date the letter
  • The fee is $20 and additional copies of the same record issued at the same time are $3. Please include a business sized self-addressed, stamped envelope. Do not send cash. We accept checks or money orders made payable to: VITAL STATISTICS and mail to:

  • Mail requests are processed in 24 to 72 hours after receipt of your request.
