Members of the decedent's immediate family may obtain a certified copies of death certificates. Relationship must be stated (i.e., I wish to obtain a copy of my father's death certificate). You must also include a copy of your CURRENT ID with the application. This office has death certificates for ALL Wisconsin deaths occurring 9/2013 to the present and for all City of Milwaukee deaths back to 1869. Uncertified death certificates usually do not include a cause of death. If you have a question, please call us at 414-286-3503.

Relationship to deceased.
Certified death certificates can only be issued to a member of the immediate family, i.e. spouse, child, parent, brother, sister or grandparent; a legal guardian of the person named on the death certificate, a representative who is authorized IN WRITING to act on behalf of the deceased, or to one who can demonstrate that the certificate is needed for the determination or protection of a personal or property right.

· Government issued employee ID card or badge with photo
· US passport
· Check or bank book

· Recent dated, signed lease
· Recent utility bill or traffic ticket
