Wisconsin Birth/Death Certificates - Credit Card Service

We can issue all Wisconsin Birth Certificates and most Wisconsin Death Certificates.  In the case of birth certificates, only those requesting a copy of their own birth certificate or those requesting a birth certificate copy of a member of his/her immediate family may obtain a one. If you are requesting a sibling's certificate, the parent's must be the same and they must have been married at the time of the birth. Stepparents cannot get a birth certificate unless you have paperwork showing custody.  Please call us if you have any questions or concerns. 

For certified death certificates, only members of the decedent's immediate family may obtain certified copies of death certificates.  Uncertified certificates usually do not include the cause of death.   Again, we have death certificates for anyone who died at a City of Milwaukee hospital, institution or residence prior to 9/2013 and we have death records for anyone who died in Wisconsin from September 2013 to the present.  YOU MUST INCLUDE A COPY OF YOUR CURRENT UNEXPIRED ID WITH YOUR APPLICATION.   If you have a question, please call us at 414-286-3503. 

Penalties: Any person who willfully and knowingly makes a false application for a birth certificate is guilty of a Class I felony [a fine of not more than $10,000 or imprisonment of not more than three years and six months, or both, per s.69.24(1). Wis. Stats.]. 

To Request a Birth or Death Certificate Using Our Credit Card Service:

You can use your credit card to order both birth and death certificates. We offer regular mail service and expedited service with FEDEX overnight delivery.   FEDEX offers overnight service to most cities, although there is always the possibility that service delays can occur.  Saturday service is also available for an EXTRA fee.

1. Please choose one of the following forms by clicking on the name of the form:

2. Print the form and fill it out, or fill it out electronically using your keyboard, then print it, sign it and FAX your request to us with a copy of your current photo ID to (414) 286-2036.  (We must be able to read the number and expiration date on your ID.)    OR

3. Fill out the form and save it to your device.   Email the form WITH a scanned copy of your current photo ID to [email protected].   (We must be able to read the number and expiration date on your ID.)    Put your name and date in the body of the email.  By emailing your request to us, your name in your email constitutes a legal signature.  

Processing Fees

The cost for the expedited service is $20.00 per record in addition to the $20.00 birth certificate or $20.00 death certificate search fee per order.  Additional copies of the same certificate, purchased at the same time as the first, cost $3.00 each. All costs must be paid by a credit card or debit card with a Mastercard or Visa logo on it. Requests for expedited service received by 2 PM CST of any day will be filled on that day.    Any check/money order payment should be made out to VITAL RECORDS.

The standard overnight mailing fee is $15.00 per order.  We use FEDEX  services.   Overnight shipping is available to most cities or addresses.   We anticipate that the certificate should get to you within 48 hours barring any service delays.   We cannot refund mailing or certificate application fees.  There may be additional shipping costs for areas outside the continental United States. The Saturday delivery fee is $12.50. This is an ADDITIONAL fee. Please be aware that if you FAX in your application on Friday and you need your certificate by the next day, you MUST check the Saturday delivery option.    If your certificate is NOT being sent to an address within the continental US, there may be additional costs for Saturday delivery.    Please be aware that Saturday delivery is not available to all US cities.    We will let you know if Saturday delivery will not be possible.

Examples:

• 2 expedited copies:  $23 for the certificates plus the $20 expediting fee plus $15 UPS fee.

• 5 copies:  $32 for the certificates sent by regular mail.

• 10 expedited copies:  $47 for the certificates plus the $20 expediting fee =  $67 plus $15 plus $12.50 if sent overnight for Saturday delivery = $94.50 total.

Again, we cannot refund mailing or certificate application fees.
