Milwaukee Health Department
The Milwaukee Health Care Partnership (MHCP) consists of local health systems such as Aurora Health Care, Ascension Wisconsin, Children's Wisconsin, Froedtert, and ThedaCare Health Inc., along with the City of Milwaukee Health Department (MHD) and the Milwaukee County Department of Health and Human Services (DHHS). These organizations work together to identify the top health needs of Milwaukee County residents through the Community Health Needs Assessment (CHNA).
The community survey includes a series of health-related questions designed to identify the top health needs facing the Milwaukee community as part of the CHNA.
Your survey participation is crucial. It helps health systems and public health groups understand what Milwaukee County residents need, so they can focus on the most important issues.
The survey is open to Milwaukee County residents aged 18 and older. We encourage all residents, especially those with children, to take the survey to ensure a wide range of opinions and perspectives.
Upon completing the survey, Milwaukee County residents (who are 18 and older) will have the opportunity to select their drawing of interest and be entered to win various prizes including:
