2015 Health Care Benefit Design Changes FAQs for City Employee Health Plan Participants

On July 22, 2014 the Common Council approved health care benefit design changes for 2015. At the same meeting, the Council also approved the expansion of the City Wellness Center to include an onsite Nurse Practitioner and Medical Assistant through Froedtert Workforce Health for 20 hours per week for employees and spouses/partners.

Transform Milwaukee Jobs Program

City of Milwaukee’s Transitional Jobs Program

In partnership with the United Migrant Opportunity Service (UMOS) agency and the Milwaukee Area Workforce Investment Board (MAWIB), the City will be placing up to 75 transitional program participants in Department of Public Works laborer assignments in August for a period not to exceed 1,040 hours.

 

To qualify for the program, participants must meet certain eligibility conditions through MAWIB: 414-270-7580 or UMOS: 414-389-6000

Important Information Regarding City of Milwaukee Wellness Program & Identify Theft Protection

The City of Milwaukee Finance and Personnel Committee will consider a resolution, file #131672, on April 16, 2014 to approve an agreement with Froedtert Health for service related to the City Wellness program that includes the resumption of the City’s “Wellness Your Choice, Milwaukee” program including the outcome based Healthy Rewards.
