Storm Water Management Plan Recertification

After a storm water management plan (SWMP) is approved, a written recertification must be submitted every five (5) years and shall be prepared by a Professional Engineer. This recertification verifies that the drainage facility is operating as originally designed and is being actively maintained as outlined in the SWMP and WDNR Post-construction Technical Standards. Pictures demonstrating the property's compliance are encouraged. The recertification includes the following:

  1. A completed SWMP Recertification Application Form (excel Application Form available here).
  2. A completed Visual Inspection for Illicit Discharges Form to determine the existence of illicit discharges or possible cross connections between storm sewer and sanitary sewer. This form is not required if the property is located in the combined sewer service area.
  3. An updated Letter of Financial Guarantee indicating that sufficient funds have been allocated to maintain the Best Management Practices as described in the approved SWMP.

SWMP recertification shall be submitted by December 31 every five (5) years from the SWMP approval date or previous recertification, whichever applies.
