Parents Nurturing and Caring for their Children (PNCC) 


What is the PNCC Program?

Caring public health nurses work with families through home visits.  This voluntary program provides prenatal care coordination services (PNCC) prenatally and up to 2 months postpartum by:

  • Working with families to identify their goals.

  • Providing information on how to care for self and baby.

  • Connecting families with community services in their neighborhood/community.

  • Provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes.


What services does PNCC offer? 

PNCC services to families include:

  • Health assessments, education and teachings

  • Goal setting and care planning

  • Advocacy

  • Home safety assessments

  • Connection to prenatal and postpartum medical providers

  • Referrals to community resources


Who is eligible to participate in the program?

All medicaid eligible pregnant women that reside in the City of Milwaukee who are interested in learning more about having a healthy baby are eligible to participate in PNCC.
