2015-2016 Community Health Assessment
The City of Milwaukee Health Department (MHD) report and analysis on current local data on city of Milwaukee residents, their health status, and the variety of features and conditions which impact their health, healthy development and quality of life. In the development of this report, the MHD was part of a regional effort led by the health system partners of the Milwaukee Health Care Partnership.
2012 Community Health Assessment
This report was commissioned by Aurora Health Care, Children’s Hospital of Wisconsin, Columbia St. Mary’s Health System, Froedtert Health and Wheaton Franciscan Healthcare in partnership with the City of Milwaukee Health Department and the Center for Urban Population Health to gather information on the health practices and health-related behavioral risks of residents.
2008 MAPP Community Health Assessment
The City of Milwaukee Health Department initiated a citywide Community Health Assessment using the Mobilizing for Action through Planning and Partnerships (MAPP) model developed by the National Association of City and County Health Officials and the Centers for Disease Control and Prevention. This process was a collaborative effort by the City of Milwaukee Health Department and a steering committee composed of leaders and representatives from a broad array of city departments, health systems, and community organizations.
