The following is a list of forms available for download from the internet. Due to the complexity of certain forms, not all permits, applications or documents are available. One can print out the form or application and mail it in. BEWARE! You must file the correct number of copies. Check the bottom of the page for the correct amount. Foreclosure Registration (RPPF) Forms

RPPF Form Instructions

Detailed instructions for completing the RPPF registration form.

RPPF Form

Online residential property foreclosure registration. You will first be guided to open an account with e-services. Then your will be guided to the form that must be filled out on-line and submitted electronically. Use this form if the foreclosure action was filed on or after October 28, 2010.

RPPF NOA (Notification of Abandonment) form

If inspection of the residential property shows the property is abandoned, the registrant must notify DNS of the abandoned state of the property. The notification shall include a description of the external condition of the property and whether the building is secure. The notification form must provide the name of the local agent or servicing company that is authorized by the foreclosure filing entity to enter upon the property and to conduct repairs or maintenance as required.

RPPF NOD (NOTICE OF DISMISSAL) form

File this Notice of Dismissal (NOD) form if the foreclosure action has been dismissed, settled, or discharged since submitting an RPPF registration form. Note: A RPPF registration form must be filed even if the foreclosure action has since been cancelled, dismissed or discharged. If that is the case, please submit BOTH the RPPF registration and this Notice of Dismissal simultaneously. This form is also used by the Abandoned Property In Foreclosure program. RPPF registrations and RPPF NOD forms apply to residential properties that had foreclosure actions filed on or after October 28, 2010. If the foreclosure action was filed prior to 10/28/10, please note that there is a separate registration and NOD form used for these properties See Abandoned Property in Foreclosure (APIF).

Abandoned Property in Foreclosure (APIF) Forms

APIF forms apply to foreclosures that were filed on or before October 27, 2010.

APIF Form

Application form used to register abandoned property in foreclosure with DNS. This form must be filled out on-line and submitted electronically. Use this form if the foreclosure action was filed on or before October 27, 2010.

Notice of Dismissal Form (APIF)

File this Notice of Dismissal (NOD) form if the foreclosure action has been dismissed, settled, or discharged since submitting an APIF registration form. Note: An APIF registration form must be filed even if the foreclosure action has since been cancelled, dismissed or discharged. If that is the case, please submit BOTH the APIF registration form and this Notice of Dismissal simultaneously. APIF registrations and APIF NOD forms apply to abandoned residential properties that had foreclosure actions filed on or before October 27, 2010. If the foreclosure action was filed after 10/27/10, please note that there is a separate NOD form used for these properties-See Residential Property Pending Foreclosure (RPPF).

Residential Rental Application Form (DNS-04)

This is the application to obtain a Residential Rental Inspection Certificate. The application will ask the owner for information about the number and location of rental units in their buildings. This is to be used in a limited part of the City otherwise known as the "UW-Milwaukee" and "Lindsey Heights" neighborhood. Vacant Building Registration Application (DNS-18A)

This is the form to be filed 30 days after a building goes vacant unless exempt as described in the Milwaukee Code of Ordinances (200-51.7) The cost to register is free and covers a 6 month period. If the building remains vacant an inspection will be made and a fee will be charged. Each subsequent 6 month period of vacancy will trigger and inspection and fee. Owners are required to maintain the interior and exterior in a safe and sanitary manner. For more details see the Vacant Building Registration brochure DNS-29A. Fire Prevention Permit Application (DNS-131)V2

Use this form to apply for a Fire Prevention Permit. Instructions are included in the application. After the application is submitted, an inspection is scheduled and the conditions indicated on the application are verified. Home Occupation Statement Application (DNS-164)V4

This form is for those who are going to use their home as a base of business operations or will be conducting business in the premises. If the premises are rented or leased, the business operator also needs to file an Owner Consent Form granting the lessee the right to use the property for business. Junk Dealers/Collectors operating out of their home should see the Junk Collectors rules and regulation brochure.

Use this form to file your exterior facade inspection survey. This form details the condition of building per the periodic inspection interval required under city ordinance. Includes listing for the professional completing the form.

Chicken Coop Site Plan Form DNS-362

This is form that must be completed by those wishing to obtain a chicken coop permit in the City of Milwaukee. See the sample below for what to put on the site plan. Bring this completed plan (DNS-362) and the Neighbor approval sheet (DNS-363) when applying for the chicken keeping permit.

Chicken Keeping Neighbor Approval Form

Title Search Form (DNS-35V2)

Use this form to request a search of Department of Neighborhood Services records for pending Special Charges and outstanding code violations. Other items included in the search are Code Compliance, Property Recording, Rent Withholding and Fire Inspection. There is a $40.56* fee per address for this search. The property owner will be responsible for any special charges assessed on the property tax bill where a Title Search was not requested in writing prior to the conveyance. Be sure to either put your fax number or name and address where you want the Title Search results returned to, somewhere on the top of the form. Send completed form along with a check/money order for $40.56* payable to the City of Milwaukee to the address listed on the top of the form. Upon receipt, the Title Search usually is processed within 3 working days. A Seller Notification Form should be submitted after every closing (or with every transfer of ownership).
