Home Visiting Programs
The City of Milwaukee Health Department offers three different home visiting programs to meet the unique needs of Milwaukee's families. These home visiting programs hope to improve prenatal, birth, and/or infant outcomes for the families served through:
Improved preventive prenatal and infant care
Timely prenatal and infant health information
Connection to community resources and support
What are MHD's three Home Visitation Programs?
Empowering Families in Milwaukee (EFM) uses evidence-based models and partners with the community to provide frequent and long-term home visits to families. The focus of EFM is pregnant women and their children who reside in zip codes: 53204, 53205, 53206, 53209, 53210, 53212, 53216, 53218, 53223, 53225 and 53233.
Nurse-Family Partnership (NFP) is an evidence-based nurse home visiting program for first-time pregnant teens and women. The nurses provide frequent and long-term home visits to families from early in their pregnancy until the child’s second birthday. The focus of the program is to improve pregnancy outcomes for the woman, to improve the child’s health and development, and to improve the economic self-sufficiency of the family
Parents Nurturing and Caring for their children is a team of nurses providing Prenatal Care Coordination (PNCC). PNCC is a Medicaid benefit that helps pregnant women get the support and services they need to have a healthy baby. A public health nurse provides home visits during the pregnancy and until the baby turns two months old.
How will the City of Milwaukee Health Department's home visiting programs impact Milwaukee's families?
These three programs hope to improve the health and well-being of participants through:
Improved prenatal care and information for pregnant women
Improved birth outcomes including healthy birth weight and gestational age
Improved infant and child health and development through preventive care coordination and child development education for families
Improved immunization status for school age children
