Residential Project Forms

Consultant Standard Disclosure Form
Consultants should provide disclosure form to homeowner when affiliated with the contractor that may perform installation work on same home, if applicable.

Residential Utility Release Form
This form is required from the homeowner for program participation. An attempt should be made to obtain records of 15 months of heating and electricity billing data from the homeowner to avoid delays in requesting from the utility. Homeowners should complete all fields on this form with the exception of the beginning and ending date fields in Section B.

Data Collection Form
All applicable shell insulation and mechanical fields should be completed. Use for both pre- and post-assessments.

Mechanical Equipment Data Collection Form-Summer Sale
Form for the HVAC Contractor to collect mechanical information for projects where replacement occurs prior to the Me² Energy Assessment. Customer must complete Me² Energy Assessment prior to receiving Summer Sale bonus. Send completed form to contractorinfo@weccusa.org or fax to (608) 442-1833.

Surveyor Additional Data Collection Form
Additional information needed to model the home.

Homeowner Report Template
A copy of a Home Performance “Your Performance Energy Report” can be used in place of the Homeowner Report.

Scope of Work Template
Boilerplate scope measures for all recommended or possible shell & mechanical improvements. The scope of work should include all shell and mechanical measures that the homeowner may install, regardless of whether they are included in the Focus on Energy Home Performance rewards package.

Installation Completion Document
Document certifying completed installation. To be signed by Homeowner, Prime Contractor and Consultant (where applicable).

Form 1 Affidavit
This form must be completed by employees seeking certification to meet the Resident Preference Program (RPP) as outlined in the Labor Standards section on page 2 of the RFQ. Employees must take the completed form, along with one of the proof of residency documents (as outlined in the Form 1 document), to one of the three organizations listed in the Labor Standards section on page 4 of the RFQ to be notarized.

Me²-Assisted Home Performance Info Sharing Disclosure Form
This form is only needed for Focus on Energy Assisted Home Performance customers (and Focus Home Heating Assistance customers) who are also participating in the Me² program. The homeowner’s signature on this form to allow Focus on Energy’s programs and Me² to share information. The homeowner is only required to submit one application to confirm eligibility for all three programs.

Wisconsin Historical Society AHI Database
Homeowners and Business Owners can check the statewide inventory database to confirm if their property is listed as a Historic Preservation Site.

Job Hours Reporting Link
Contractors are required to report all hours worked on each project within 10 business days of completing the project through this portal. For commercial projects where Davis Bacon wages are required to be paid, contractors would report the hours worked on weekly certified payroll reports rather than using the Job Hours Reporting platform (see Form WH-347 Certified Payroll Report).

Me² participating Contractors and Consultants are required to meet program criteria. Please note that the Me² program (including WECC and the City of Milwaukee) does not endorse or guarantee the work or services of any particular Consultant or Contractor, but maintains a directory of Consultants and Contractors who have applied and been authorized to participate in the program. Participating Consultants are independent Consultants who have been accredited and certified by the Building Performance Institution. Participating Contractors are independent Contractors who have agreed to specified program requirements. While a list of participating Consultants and Contractors have been screened and qualified to the best of our ability, you will be entering into direct relationships with the Consultant or Contractor of your choice and it is important that you review their credentials, qualifications, and references to make an informed decision. Consultants and Contractors who participate in the program warrant their own work and may charge different fees for services. It is generally good practice to contact several Contractors before selecting one. (Please see the full program disclaimer on the directory of  Consultants and Contractors.)
