Forgery packet: This packet contains the forms necessary for you to file a FORGERY complaint under Wisconsin State Statute 943.38 with the Milwaukee Police Department. ONLY those incidents occuring WITHIN the City of Milwaukee will be accepted. - Click here for PDF download

Worthless Check packet: This packet contains the necessary forms and instructions for you to proceed with the filing an Issue of Worthless Checks (IOWC) complaint under Wisconsin State Statue 943.24. Only those checks received by you within the City of Milwaukee can be filed with the White Collar Crimes Unit.
