Police reports are available through the Milwaukee Police Department Open Records Section.

Traffic Accident and Crash Reports

The Milwaukee Police Department no longer maintains or provides copies of Wisconsin MV4000 accident reports. See Wisconsin Crash Reports.

Reportable Traffic Accident Reports - City of Milwaukee


"Reportable" traffic accident reports, completed by Milwaukee Police Department (MPD), will be available from MPD Open Records Section, generally 10 days after the incident. To obtain a report:

Search online or call to verify that your report is completed by MPD and is available. Please have case number, name or date of accident for verification.

Non-reportable Accident Forms - City of Milwaukee


Milwaukee Police Department (MPD) may retain some documents pertaining to accidents in which law enforcement investigation was necessary (i.e. fatal accidents, supplements to accident reports, OWI reports).

A non-reportable accident is any accident that does not:

  1. result in injury or death to any person,
  2. damage government-owned non-vehicle property to an apparent extent of $200 or more,
  3. result in total damage to property owned by any one person to an apparent extent of $1000 or more.

Crash Reports (MV4000) - State of Wisconsin


All MV4000 accident reports, completed by MPD officers, will be available from the Wisconsin Department of Transportation (WisDOT) Division of Motor Vehicles (DMV) Accident Records Unit, generally 10 days after the incident.
