Death Certificates By Mail

City of Milwaukee Health Department Vital Statistics

Members of the decedent's immediate family may obtain a certified copies of death certificates. Relationship must be stated (i.e., I wish to obtain a copy of my Grandfather's death certificate).


To request a death certificate by mail, write a letter including the following information:

  • Relationship

  • Reason the certificate is needed

  • Full name of person named on the record.

  • Date of death.

  • City and county of death

  • Include a copy of an acceptable form of identification.    These are:'

ONE of these:

·         Wisconsin driver’s license
·         Wisconsin photo ID
·         Out-of-state driver’s license or photo ID card
 
OR
 
TWO of these:
·         Government issued employee ID card or badge with photo
·         US passport
·         Check or bank book
·         Major credit card
·         Health insurance card
·         Recent dated, signed lease
·         Recent utility bill or traffic ticket

 

  • Be sure to sign and date the letter
  • The fee is $20 and additional copies of the same record issued at the same time are $3. Please include a business sized self-addressed, stamped envelope. Do not send cash. We accept checks or money orders made payable to: VITAL STATISTICS and mail to:

  • Mail requests are processed in 24 to 72 hours after receipt of your request.
