In the case of birth certificates, only those requesting a copy their own birth certificate or those requesting a birth certificate copy of a member of his/her immediate family may obtain a one.  For death certificates, only members of the decedent's immediate family may obtain certified copies of death certificates.

To Request a Birth or Death Certificate Using Our Credit Card Expedited Service:

1. Please use the birth certificate fax application form or death certificate fax application form by clicking on the name of either.
2. Print the form and fill it out, or fill it out electronically using your keyboard, then print it.
3. Fax your request to us at (414) 286-2036 along with a copy of your current identification.

Processing Fees

The cost for this expedited service is $20.00 per record in addition to the $20.00 birth certificate or $20.00 death certificate search fee per order.  Additional copies of the same certificate, purchased at the same time as the first, cost $3.00 each.  All costs must be paid by a credit card or debit card with a Mastercard or Visa logo on it.  Requests received by 3 PM CST of any day will be filled on that day.

The standard overnight mailing fee is $15.00 per order.  There may be additional shipping costs for areas outside the continental United States or for Saturday delivery.  Standard overnight service will be used for these requests, unless otherwise indicated on your application.  Please be aware that if you FAX in your application on Friday and you need your certificate by the next day, you must check the Saturday delivery option.

Examples:

• 2 copies:  $23 for the certificates plus the $20 expediting fee = $43 total if sent by regular mail.

• 5 copies:  $32 for the certificates plus the $20 expediting fee = $52 plus $15 if sent overnight on a week day = $67 total.

• 10 copies:  $47 for the certificates plus the $20 expediting fee =  $67 plus $15 plus $12.50 if sent overnight for Saturday delivery = $94.50 total.
