In the case of birth certificates, only those requesting a copy their own birth certificate or those requesting a birth certificate copy of a member of his/her immediate family may obtain a one. For death certificates, only members of the decedent's immediate family may obtain certified copies of death certificates.

Penalties: Any person who willfully and knowingly makes a false application for a birth certificate is guilty of a Class I felony [a fine of not more than $10,000 or imprisonment of not more than three years and six months, or both, per s.69.24(1). Wis. Stats.].

 To Request a Birth or Death Certificate Using Our Credit Card Service:

1. You can use your credit card to order both birth and death certificates. We offer regular mail service and expedited service with UPS overnight delivery. Saturday service is also available for an EXTRA fee.

2. Please choose one of the following forms by clicking on the name of the form:

   • expedited birth certificate FAX application form

   • birth certificate FAX application form

   • expedited death certificate FAX application form 

   • death certificate FAX application form

3. Print the form and fill it out, or fill it out electronically using your keyboard, then print it.

4. Fax your request to us at (414) 286-2036.

Processing Fees

The cost for the expedited service is $20.00 per record in addition to the $20.00 birth certificate or $20.00 death certificate search fee per order.  Additional copies of the same certificate, purchased at the same time as the first, cost $3.00 each. All costs must be paid by a credit card or debit card with a Mastercard or Visa logo on it. Requests for expedited service received by 3 PM CST of any day will be filled on that day.

The standard overnight mailing fee is $15.00 per order. There may be additional shipping costs for areas outside the continental United States. The Saturday delivery fee is $12.50. This is an ADDITIONAL fee. Please be aware that if you FAX in your application on Friday and you need your certificate by the next day, you MUST check the Saturday delivery option. Examples:

• 2 expedited copies:  $23 for the certificates plus the $20 expediting fee plus $15 UPS fee.

• 5 copies:  $32 for the certificates sent by regular mail.

• 10 expedited copies:  $47 for the certificates plus the $20 expediting fee =  $67 plus $15 plus $12.50 if sent overnight for Saturday delivery = $94.50 total.
