DIRECT DEPOSIT OF CITY PAYCHECKS
All regularly appointed City of Milwaukee employees must use Direct Deposit. Complete the following form, then submit it to the Comptroller's Office (see bottom of application), along with a copy of one of the following documents:
- blank, voided check from your checking account (no starter checks)
- a signed memo on institution letterhead for a savings account

- Each time you change banks or accounts, you must complete and submit the form above, along with a blank, voided check or signed memo (see above).
